ON-SCREEN TEXT: UnitedHealthcare Choice Plus Premier Provider Network – Sandia Health Savings Plan with Health Savings Account

Thank you for taking the time to learn more about your benefits and the UnitedHealthcare Choice Plus Premier Provider Network offered by UnitedHealthcare and Sandia National Laboratories.

ON-SCREEN TEXT: Health Savings Account (HSA) No federal income tax

Savings grow tax-free No taxes on earnings

The Health Savings Plan is a qualified high-deductible health plan, which allows you to open a Health Savings Account, known as an HSA. An HSA is a unique savings account. You don’t pay federal income tax on the money you deposit into it or on money you withdraw to use for qualified medical expenses.

Plus, your savings grow tax-free. If you invest your HSA dollars in the available options with Optum Bank®, you don’t pay taxes on the earnings from your investments.

ON-SCREEN TEXT: Tax advantages* Build savings for retirement

*Some states do not recognize HSA contributions as a taxable deduction, and some states tax interest earned on your HSA. Your own HSA contributions are pre-tax. See IRS Publication 969, or consult a qualified tax advisor to see how your state treats HSA contributions. California and New Jersey don’t allow income tax deductions for HSA contributions. New Hampshire and Tennessee tax HSA interest and investment earnings.

Please read your plan documents. Additional information such as benefit details, plan limitations and exclusions, and the costs of coverage can be found in the Benefit Booklet.

The UnitedHealthcare plan with Health Savings Account (HSA) is a qualifying high deductible health plan (HDHP) that is designed to comply with IRS requirements so eligible enrollees can open a Health Savings Account (HSA) with Optum Bank®®, Member of FDIC, a subsidiary of Optum Financial. The HSA refers only and specifically to the Health Savings Account that is provided in conjunction with a particular bank, such as Optum Bank®, and not to the associated HDHP.

An HSA has tax advantages and it can help you build your savings for retirement.

ON-SCREEN TEXT: Paying for network care

Here’s how paying for network care works with an HSA.

ON-SCREEN TEXT: Common health terms

Let’s start by going over a few common health terms.

ON-SCREEN TEXT: Deductible

*Annual deductibles for Tier 1 and Tier 2 apply towards each other, In- and out-of-network does not. Annual out-of-pocket maximum for Tier 1 and Tier 2 apply towards each other, in- and out-of-network does not. For family coverage, no one receives the plan benefits until the total family deductible is met.

Your deductible is the amount you’ll need to pay before your plan starts to pay for covered services. This includes prescription drug costs.

ON-SCREEN TEXT: Coinsurance

Coinsurance is the percentage you pay as your share of a covered health care service.

ON-SCREEN TEXT: Out-of-pocket limit

The out-of-pocket limit is the most you could pay during a coverage period (January through December) for your cost share of covered services. After you meet this limit, the plan will usually pay one hundred percent of the allowed amount.

ON-SCREEN TEXT: Please read your plan documents. Additional information such as benefit details, plan limitations and exclusions, and the costs of coverage can be found in the Benefit Booklet.

ON-SCREEN TEXT: Paying for network care

You pay for all services, including prescriptions, until you meet your deductible. And you can use your HSA to help pay these expenses.

After you reach the deductible, you share the costs with the plan. This is known as coinsurance. You can use an HSA to help pay your share. Plus, using a Tier 1 provider can save you money.

This example assumes employee-only coverage and deductible has been met. With a 200-dollar doctor visit, if the coinsurance for a Tier 1 provider is 10 percent, you pay 20 dollars. If the coinsurance for a Tier 2 provider is 20 percent, you pay 40 dollars. If the coinsurance for a Tier 3 or out-of-network provider is 40 percent, you pay higher out-of-pocket costs, 80 dollars.

Once you reach your out-of-pocket limit, you’re done paying and your plan pays one hundred percent of covered services for the rest of the year.
